Red marks and dark spots from acne
The flat red or brown spots a breakout leaves behind usually aren't scars. Red marks (PIE) are small blood vessels left widened by inflammation, and brown marks (PIH) are extra pigment. Neither involves lost collagen. Both often fade over months with daily sunscreen, and they respond to pigment-focused care such as PicoWay Resolve, not collagen remodeling.

Are red or brown acne marks actually scars?
Usually not, and knowing the difference can spare you an expensive treatment you don't need. A scar is a change in the skin's surface, where collagen was lost. A mark is a change in color, with the surface still level. Neoderma finds that much of what people call acne scarring belongs in the second group.
Flat mark
Red marks (PIE)
A flat pink or red spot where a pimple healed. The surface stays level: it is leftover widened capillaries, not lost tissue, so it isn't a true scar.

Flat mark
Brown marks (PIH)
A flat brown or tan spot: extra pigment laid down during the inflammation. It is more common, and slower to fade, on deeper skin tones.

Not sure whether yours are flat? Under a single light from the side, flat marks fade from view as you turn your head, while indented scars cast small shadows. How to tell scars from marks
What are red acne marks (PIE)?
PIE stands for post-inflammatory erythema: the flat pink or red spot that stays behind where a pimple used to be. The inflammation left the tiny blood vessels in that spot widened, and the color you see is those vessels showing through. Nothing is missing from the skin, and the surface stays level, which is why a red mark isn't a true scar.
Red marks often take months to settle, but many fade by themselves in that time.
What helps red marks fade?
Patience comes first, then gentle care. Neoderma's order of play for red marks is time and daily sunscreen, followed by gentle resurfacing and calming facials when they're needed. Aggressive treatment isn't the answer for PIE.
The LED mask is one calming option: Neoderma uses it to calm redness as well as to treat acne, as a finishing step after facials or as a $50–$75 add-on.
What are dark spots from acne (PIH)?
PIH stands for post-inflammatory hyperpigmentation: a flat brown or tan spot where a breakout was. During the inflammation, pigment cells in the area became overactive and laid down extra melanin, and that deposit is what stays visible after the spot heals.
PIH is more common in deeper skin tones, and it tends to last noticeably longer there. If that describes your skin, acne scar treatment for darker skin covers how treatment is planned around it.
How long do acne marks last?
Usually months rather than weeks, for red and brown marks alike, and many fade on their own in that time. What slows them down most is the sun: UV exposure keeps darkening post-acne marks and can wipe out weeks of progress. That makes daily sunscreen part of the treatment rather than an extra, and in Southern California that means every day of the year.
What treats dark spots from acne?
Pigment-focused care, always alongside daily sunscreen. Neoderma matches brown marks to three things:
- PicoWay Resolve. A picosecond laser that works photoacoustically, with minimal heat. Its Resolve handpiece divides the beam into 100 micro-beams that reach beneath the surface to break up pigment, while the outer layer of skin stays intact. Neoderma calls it its first choice for pigment. Sessions are $200–$600, depending on the size of the area.
- A Vi Peel. A medical-grade chemical peel of about 20 minutes, followed by roughly a week of light peeling.
- Pigment-regulating skin care. A medical-grade home regimen from SkinBetter Science or ZO Skin Health, matched to what a 3D skin analysis finds. SkinBetter's Even Tone Correcting Serum, for one, brightens dark spots; medical-grade skin care for acne has the rest.
Recovery from PicoWay is usually easy: brief redness that a cold compress soothes. If your skin darkens easily, your provider may first have you prepare it with a topical pigment inhibitor. How many sessions your marks need is set by your provider after seeing your skin.
How do I stop new marks from forming?
Every mark started as an inflamed breakout, so the most direct prevention is getting active acne under control. The less time a spot stays inflamed, the less lasting damage it does; for cystic acne, Neoderma's Kenalog injections calm an inflamed cyst quickly. For ongoing breakouts, see acne facials and peels.
The other half is sunscreen. UV keeps darkening marks that would otherwise fade, so wearing SPF daily protects the progress you've made.
If, under side light, your marks turn out to have depth, they're scars rather than marks, and acne scar treatment is the next page to read.
Acne mark questions
Will a laser make dark acne marks worse?
Heat is the concern: lasers that work with light and heat bring a higher chance of burns and downtime, particularly on darker skin. PicoWay Resolve works photoacoustically with minimal heat, and if your skin darkens easily, your provider may ask you to use a topical pigment inhibitor beforehand.
Are red marks and brown marks treated the same way?
No. Red marks start with time and daily sunscreen, then gentle resurfacing and calming facials if needed. Brown marks get pigment-focused care: PicoWay Resolve, a Vi Peel and pigment-regulating skin care.
Do flat marks need scar treatment like Secret RF?
Usually not. Collagen remodeling such as Secret RF microneedling is for indented scars; flat marks have no missing collagen, so they respond to pigment-focused care instead.
